Teaching practice within a thriving social enterprise delivering integrated health and social care
- Established, innovative and highly regarded APMS practice
- List size 5,000 and growing
- High achievement in prescribing, QOF scores and patient access
- Strongly motivated and experienced nurse team dealing efficiently with chronic care
- Based in integrated service centre including Library, Café, Children's Centre, Church, social care and wellbeing work
- Experienced GP required to further practice development and challenge health inequality in an area of social deprivation
- Provide clinical leadership of the practice and high quality delivery of clinical services
- Five to six sessions to include clinical governance
- Weston-super-Mare, 10 minutes from junction 21 of the M5
- 40 minutes from Bristol or Taunton
